The Palliative Care Team

St. Dominic Hospital offers a specialized team to assist the physician in providing care to patients with serious, progressive illness, while helping them make the most of life. The Palliative Care Team is a consultation service that is designed to complement the care you provide to patients. Our Palliative Care Coordinator serves as the initial point of contact for physicians and patients, working with them to assess which other members of the team to involve.

Questions about Palliative Care

Why palliative care?
The purpose of the palliative care program is to help patients and their families through the course of their serious, progressive or life-threatening illness. Palliative care focuses on relief of the pain, symptoms and stress of serious illness. The goal is to help people live comfortably and to provide the best possible quality of life for them. We encourage patients and their families to actively participate in identifying their needs and developing a plan of care to meet those needs.

When is the right time for palliative care?
Palliative care is helpful through all stages of illness. It is best introduced early and can be provided at the same time as curative treatments.

Is palliative care the same as hospice care?
No. Hospice care is available for those patients with limited life expectancy, while palliative care is appropriate for any stage of a serious illness. Patients may choose to transition to a hospice program at a later date.

Refer a patient for Palliative Care at St. Dominic's

Physicians can consult the Palliative Care Team by writing an order on a patient’s chart or via St.Dominic’s electronic ordering system.  If anyone other than the physician refers the patient, the Palliative Care Coordinator will screen the patient and contact the physician to request an order prior to initiating the consultation.
