Diabetes Program Information for Referring Physicians

St. Dominic's Diabetes program is patient centered and patient focused.  Our certified diabetes educators offer two educational classes for patients; Eating Well with Diabetes and LivingWell with Diabetes. 

Eating Well with Diabetes

This class focuses on:

  • Nutritional counseling
  • Portion control
  • Food content

The patient will learn healthy receipes and will become equipped with the basic skills to make food choice to him/her well.  

LivingWell with Diabetes

This class covers:

  • Disease process
  • Possible complications related to disease progression
  • Foot care
  • Access and management of medical supplies
  • Importance of healthcare provider follow-up

This class also includes a focus on:

  • Medication education
  • The purpose, rationale and technique for monitoring methods
  • Proper technique for insulin injection
  • Hyperglycemia and hypoglycemia management
  • Sick day protocols

These classes are usually group sessions with one-on-one sessions available if medically necessary.  St. Dominic's diabetes program is accredited by the American Diabetes Association.  St. Dominic's diabetes coordinator will provide you with a simple referral sheet that you can fax to our office.  A physician signature is required for certification purposes.
